허벅지 둘레와 제2형 당뇨병 및 심혈관계질환의 위험인자 간 상관관계와 허벅지 둘레 증가를 위한 운동 중재 방법의 고찰

Abstract

PURPOSE

Metabolic diseases such as insulin resistance, type 2 diabetes and cardiovascular disease are associated with dysfunction of glucose metabolism. Thigh circumference reflect body muscle mass and peripheral subcutaneous fat. Based on previous studies, thigh circumference is associated with reduced cardiometabolic dysfunction. The purpose of this review is to investigate the correlation between cardiometabolic dysfunction and thigh circumference, and to provide guidelines for exercise intervention methods for increasing thigh circumference.

METHODS

In this review, we summarized previous studies investigating the correlation between the thigh circumference and cardiometabolic disease and also examined the previous study about the change of thigh circumference by exercise.

RESULTS

The skeletal muscles that constitute the human thigh have a positive effect on glucose metabolism, and the subcutaneous fat constituting the thighs also plays a positive role in the body, unlike the visceral fat of the abdomen. In the case of women, inconsistent results were observed in the thigh circumference by exercise intervention, whereas significant results were found in men with increased thigh circumference by exercise intervention.

CONCLUSIONS

Exercise-induced increase in thigh circumference may have a positive impact on lowering the prevalence of type 2 diabetes and cardiovascular disease. However, there were some limitations in the study of previous studies on the change of thigh circumference by the exercise intervention. It seems that gender-associated differences exist in the thigh circumference by exercise training.Therefore, additional studies on effective exercise interventions for increasing thigh circumference may be necessary.

서 론

현대사회가 고령화 사회로 진입함에 따라 대사질환 및 이에 따른 심혈관계질환 발생이 증가하고 있으며, 2016년 통계청 자료에 따르면 대한민국 국민의 사망원인은 암, 심장질환, 뇌혈관질환 순으로 나타나고 있다. 제2형 당뇨병(type 2 diabetic mellitus) 또한 사망원인 6위에 해당하며(통계청), 2016년 기준 대한민국 국민의 약 236만 명이 갖고 있는 질환으로 나타났다(건강보험심사평가원). 당뇨병환자의 가장 흔한 사망원인은 심혈관계질환으로 당뇨병이 없는 사람에 비해 남자의 경우에는 2-3배, 여자의 경우에는 3-5배 위험인자가 높은 것으로 나타났다[1-4].
노화가 진행됨에 따라 골격근의 양이 점차 줄어드는 근감소증(sarcopenia)의 진행은 피할 수 없으며, 이러한 결과로, 인슐린 저항성(insulin resistance, IR), 대사증후군(metabolic syndrome), 제2형 당뇨병과 같은 만성질환의 위험성이 증가되어 건강 위험요인으로 발전하게 된다[5,6]. 인슐린 저항성은 혈당을 낮추는 인슐린의 기능이 떨어져 세포가 글루코스(glucose)를 효과적으로 연소하지 못하는 것을 말한다. 인슐린 저항성이 오랫동안 지속될 경우, 인체는 너무 많은 인슐린을 만들어내고 이로 인해 췌장의 β-세포의 기능이 저하되어 제2형 당뇨병으로 발전하게 되며, 이는 고혈압, 고지혈증(hyperlipidemia), 심혈관계질환 등의 합병증을 초래한다[7,8].
영양 과잉 섭취와 신체활동부족으로 인해 나타나는 비만, 특히 복부 비만은 인슐린 저항성과 다양한 대사 위험 인자에 중요한 역할을 한다[9]. 이미 여러 선행연구를 통해 복부지방과 허리 둘레(waist circumference, WC) 간에 양의 상관관계가 보고되었고[10-12], 이에 따라 WC 또는 허리/엉덩이 둘레 비율(waist to hip ratio, WHR)은 대사성 질환의 예후를 측정할 수 있는 실제 인체 측정 기준표로 사용되고 있다[10-12].
골격근은 인체에서 가장 많은 부분을 차지하며, 인슐린에 반응하여 글루코스 섭취(uptake)에 가장 주요한 역할을 하는 조직이기 때문에[13-15] 골격근량의 감소는 인슐린 민감성(insulin sensitivity)에 영향을 미치고 제2형 당뇨병과 같은 대사 장애를 촉진시킬 수 있다. 따라서 골격근량을 증가시키거나 유지하는 데 도움을 주는 저항성 운동과 같은 중재 방법은 대사성 질환을 예방하는 데 매우 중요하다고 여겨진다. 또한 우리 몸에서 허벅지 근육은 총 골격근량의 30-40%를 차지하기 때문에[16], 허벅지 근육의 질량 증가와 근력 향상을 위한 운동 중재 방법의 정립이 대사 및 심혈관계질환 위험인자 감소에 효과적일 것으로 판단된다. 선행연구를 살펴보면 허벅지 구성성분이 성별에 따라 차이가 있는 것으로 보고되었지만[17,18]. 현재까지 남녀 간 허벅지 둘레의 구성성분의 차이를 고려한, 적절한 운동형태와 운동강도 설정에 관한 연구는 매우 부족한 실정이다.
이에 본 종설(review)에서는 허벅지 둘레와 대사 및 심혈관계질환과의 상관관계를 알아보고, 운동 중재 방법을 통한 허벅지 둘레의 변화에 관한 선행연구를 성별에 따라 비교·검토하고자 한다.

본 론

1. 인슐린 저항성과 허벅지 둘레의 상관관계

선행연구결과들을 분석하여 허벅지 둘레에 영향을 미치는 주요 구성인자를 파악해본 결과, 남성의 경우 대부분 피하지방과 근육을 포함하고 있고, 여성은 많은 부분이 피하지방으로 구성되어 있는 것으로 보고되었다[17,18]. 남성의 허벅지에 위치하는 피하지방은 글루코스 대사 및 인슐린 저항성 개선에 긍정적 결과를 초래하여, 혈중 지질농도에도 긍정적인 영향을 미친 것으로 파악되었다[17,18]. 이러한 결과를 바탕으로 허벅지 둘레의 주요 구성인자인 근육과 지방은 각각 어떤 역할을 하는지 살펴보고자 한다.
인간의 체중의 35-40%를 차지하는 골격근은 글루코스 대사과정에서 췌장에서 분비된 인슐린(insulin)의 신호를 받아, 글루코스를 흡수하여 에너지원으로 사용하는 역할을 하며[19,20], 유리지방산(free fatty acid, FFA)의 산화(oxidation)를 위한 주요 부위이다[19,21]. 따라서 앞서 언급했듯 근육량의 증가는 인슐린 민감성(insulin sensitivity) 개선에 효과적이라는 연구들이 보고되고 있다[12-14].
또한 연세대 보건대학원 연구팀이 2009년에서 2011년에 걸쳐 한국의학연구소(Korea Medical Institute)에서 건강검진을 받은 30-79세 남녀 약 32만 명을 대상으로 파악한 결과[22], 허벅지가 가는 사람은 당뇨병에 걸릴 위험이 높다는 상관관계를 보고하였다. 이 연구에서 허벅지 둘레가 1 cm 줄어들 때마다 당뇨병 위험이 남성은 8.3%, 여성은 9.6%씩 증가한다고 제시하고 있다[22]. 이러한 연구결과들은 허벅지의 근육량과 인슐린 저항성, 혈당, 혈청 지질농도와 같은 대사질환 및 심혈관계질환 위험요소 간 부적 상관관계를 뒷받침한다[21,23,24].
이렇듯 허벅지 근육이 인간의 대사 작용에 긍정적 결과를 가져온다는 연구들이 보고되고 있다. 그렇다면 우리 몸에서 허벅지에 위치하는 지방은 어떤 역할을 할까? 복부와 대퇴부의 지방은 지질분해 활성에 서로 다르게 작용한다고 알려져 있다. 복부의 내장지방(visceral adipose tissue)은 지질대사 및 글루코스 대사이상 등의 심혈관계 질환의 위험인자와 밀접한 관련이 있음이 보고된 반면[25-27], 허벅지의 지방은 주로 피하지방(subcutaneous adipose tissue)으로 구성되어 있으며, 피하지방은 FFA를 흡수하여 체내 인슐린 저항성을 줄이고 간, 췌장, 골격근육, β-세포를 보호한다[17,18]. 또한 복부 내장지방과는 대조적으로, 허벅지 지방은 지방산을 순환시키기 위한 ‘대사성흡수원(metabolic sink)’ 역할을 하는 것으로 보고된 바 있으며[28]. 피하지방은 동맥경화를 예방하는 효과가 있다는 연구결과도 있다[29,30]. 이러한 연구들은 허벅지의 피하지방이 복부의 내장지방과는 다르게 인간의 건강에 긍정적인 영향을 끼칠 가능성이 있음을 시사한다.

2. 제2형 당뇨병 및 심혈관계질환과 허벅지 둘레의 상관관계

우리 몸의 기관들은 혈관을 통해 산소와 영양공급을 원활하게 함으로써 그 기능을 유지할 수 있다. 하지만 이때 혈관의 기능, 특히 혈관 확장(vasorelaxation) 기능이 떨어져 혈류량(blood flow) 공급이 충분히 이루어지지 않는다면 각 기관들은 제 기능을 할 수 없게 되어 각 장기의 부전(dysfunction)이 나타난다[25,26]. 특히 이러한 현상은 심혈관계통에 뚜렷하며 장기간 진행될 경우 심혈관계질환으로 발전하게 된다[25,26]. 일반적으로 흡연, 신체활동부족, 비만, 노화로 인해 혈관의 내피세포(endothelial cell)기능이 저하되면서 심혈관계질환의 전조증상으로 나타나고 정기적으로 지속될 경우 동맥경화의 주된 원인이 된다[31,32]. 신체조성과 심혈관계질환 위험인자 간의 다양한 연구결과를 살펴보면, WHR이 높으면 혈중 글루코스 수준이 높고, 제2형 당뇨병 발병이 증가하며[33], 높은 수준의 WC는 심혈관계질환에 의한 사망률을 더 높게 예측할 수 있다고 보고되고 있다[34-36]. 허벅지 둘레는 허리 둘레나 다른 인체부위의 측정 둘레보다 골격근량을 잘 반영하는 것으로 나타났으며[36], 신체 둘레 중 허벅지 둘레가 감소할수록 당뇨병, 고지혈증, 고혈압과 같은 질환의 발생률이 증가한다고 보고되고 있다[17,19,21,23,38]. 허리 둘레 및 신체질량지수(body mass index, BMI)와는 독립적으로 허벅지 둘레가 가는 사람은 남녀 모두 심혈관계질환 관련 사망률이 높다고 보고하고 있으며 많은 연구결과들이 일치하고 있다[39]. 또한 허벅지 둘레가 엉덩이 둘레보다 심장병 위험요인과 관련이 있다는 보고들이 있으며[38,40], 허벅지 둘레가 증가할수록 허혈성 심장질환의 위험도를 낮춘다는 연구결과도 보고되었다[38]. 하지만 이러한 대부분의 연구들이 각 인자들의 상관관계만을 분석/제시하여 허벅지둘레와 심혈관계질환과의 직접적 인과관계를 밝혀내지는 못한 실정이다. 이는 허벅지 둘레라는 특성으로 인해 동물을 이용한 기전(mechanism) 연구가 불가능하여 생긴 이유로 사료된다.
또 다른 연구에서는 허벅지 둘레와 당뇨병 유병률과의 관련성을 분석한 결과, 남녀의 허벅지 둘레가 작아질수록 당뇨병 유병률이 증가함을 보고하였고(남자는 2.9배, 여자는 4.1배) [41], 허벅지 둘레는 폐경 후 여성과 갱년기 여성에서 심혈관계 질환과의 유의한 관련성을 보고하였다[42]. 또한 한국의 30-79세 남녀를 대상으로 한 연구에서, 가는 허벅지 둘레와 당뇨병과의 연관성에서 유의미한 결과를 보고하며 허벅지 둘레가 당뇨병의 마커일 수 있다는 가능성을 제시하였다[43].

3. 운동 중재를 통한 여성의 허벅지 둘레 변화

본 장에서는 남성과 여성의 허벅지 둘레를 구성하는 요인의 차이를 고려하여[17,18], 운동 중재를 통해 여성의 허벅지 둘레 변화 양상을 검토하였다. 또한 허벅지 둘레를 구성하는 골격근과 피하지방이 심혈관계질환 및 대사질환 예방 및 개선을 위해 체내 긍정적 영향을 미칠 수 있다는 결과를 토대로[17-21,23,28], 허벅지 둘레 증가를 위한 효과적인 운동 중재 방법은 무엇인지 살펴보았다. 비만여성노인을 대상으로 진행한 한 연구에서는 유산소 운동, 저항성 운동, 복합 운동 그룹으로 나누어 12주간의 트레이닝을 진행한 결과, 유산소 운동 그룹과 저항성 운동 그룹은 각각 허벅지 둘레가 감소하는 경향을 보였으며, 복합 운동 그룹(유산소+저항성)에서만 허벅지 둘레가 유의하게 증가했다[44]. 이 연구에서는 저항성 운동 프로그램은 하지 운동 프로그램뿐만 아니라 상지 근력 운동이 상당수 포함되어 있었기 때문에, 향후 허벅지 저항성 운동 위주의 프로그램 처치를 통한 허벅지 둘레 변화에 대한 추가 연구가 필요하다고 사료된다. 또한 비만여중생을 대상으로 12주간의 뉴 스포츠 운동 프로그램을 진행한 연구에서는 허벅지 둘레가 감소했으며[45], 고령여성을 대상으로 12주간의 에어로빅댄스 운동 프로그램을 진행한 연구에서도 허벅지 둘레의 감소가 나타났다[46]. 뉴 스포츠 운동 프로그램의 경우, 피험자에게 개별적인 운동 강도의 처치가 명확히 행해지지 않았으며, 뉴 스포츠 운동 프로그램 중 근력 향상에 효과적인 것으로 알려진 플링고(flingo) 운동 프로그램을 주 1회 포함하였으나[45], 이 역시 허벅지 둘레 증가를 위한 운동 프로그램은 아니었다고 판단된다. 하지만, 65세 이상의 여성들을 대상으로 한 다른 연구에서는 12주간의 수중 운동을 실시한 결과, 허벅지 둘레의 유의한 증가를 확인할 수 있었다[47]. 여성을 대상으로 한 대다수의 연구들이 허벅지 둘레의 증가 유무를 파악하기 위한 의도가 아닌 신체 구성 변화를 파악하기 위함이었으며, 다수의 선행연구에서 저항성 운동 프로그램을 처치할 때 몸 전체의 근력운동을 구성하였기 때문에 허벅지 둘레 변화에 대한 운동의 효과를 명확히 해석하기 어려운 측면이 있다. 반면에, 수중운동을 실시한 그룹에서는 허벅지를 주로 사용하는 운동 프로그램들로 구성되어 이와 같은 결과를 가져온 것으로 보인다[47]. 선행연구결과를 참고하여 운동 유형, 운동 강도 등에 따른 여성의 허벅지 둘레의 변화를 Table 1에 요약 제시하였다.

4. 운동 중재를 통한 남성의 허벅지 둘레 변화

여성집단과는 다르게, 운동 중재를 통한 남성의 허벅지 둘레 변화는 다음과 같은 결과가 나타났다. 6개월간 운동에 참여한 적이 없는 남자 대학생을 대상으로 12주간의 저항성 운동을 수행한 연구에서 허벅지 둘레가 증가하는 경향을 보였다[48]. 또한 남자 대학생 20명을 대상으로 운동 강도에 따라 각 10명씩 1-repetition maximum (1RM)의 80%그룹과 1RM의 50%그룹으로 나누어 12주간의 저항성 운동을 수행한 결과 1RM의 80%그룹에서 허벅지 둘레가 유의하게 증가했으며, 1RM의 50%그룹에서는 증가하는 경향을 보였으나 통계적으로 유의하진 않았다[49].
또한 중년남성을 대상으로 가압(blood flow restricted training, BFRT) 저항성 운동그룹과 저항성 운동그룹으로 나누어 10주간 운동을 수행한 연구에서는 두 그룹 모두 허벅지 둘레가 유의하게 증가하였고, 집단 간 비교에서는 가압 저항성 운동그룹에서 허벅지 둘레의 유의한 증가를 확인할 수 있었다[50]. 또한 남자 대학생을 세 그룹(저항성 1RM 65%/가압 저항성 1RM 20%/가압 저항성 1RM 65%)으로 나누어 8주간의 운동을 수행한 연구에서는 세 그룹 모두 허벅지 둘레가 유의하게 증가하였다[51]. 이 연구에서 운동 프로그램 구성은 3가지 운동을 모두 허벅지(하지) 운동을 처치하였으며, 허벅지 둘레는 <가압 저항성 1RM 65%-가압 저항성 1RM 20%-저항성 1RM 65%> 순으로 유의하게 증가한 것으로 나타났다[51]. 또한 남자 엘리트 축구선수를 대상으로 가압 트레이닝 걷기(유산소)를 13주간 수행한 결과 허벅지 둘레가 유의하게 증가한 것으로 나타났다[52]. 마지막으로 남자 대학생을 대상으로 1회성의(acute) 가압 저항성 운동(SP, systolic blood pressure+1RM 30%)과 저항성 운동(1RM 60%) 집단으로 나누어 수행한 연구가 있었다. 가압처치 시에 각 피험자의 SP에 맞추어 혈류를 제한하는 방법을 사용하여, SP의 강도에 따라 다시 세 집단으로 나누어 운동이 수행되었다. 연구에서는 가압 저항성 운동을 수행한 두 집단(SP+1RM 30%, SP 200%+1RM 30%)에서 1회의 운동 직후 허벅지 둘레의 유의한 증가를 확인할 수 있었다[53].
가압 트레이닝은 공압식 전용 벨트(가압 벨트)또는 압력 커프(cuff)를 사용하여 처치하고자 하는 신체부위에 압력을 가하여 근육의 국소 빈혈(ischemia)을 통해 정맥 혈류의 공급을 제한하는 방법으로 낮은 강도의 훈련으로도 근력 강화와 근 비대를 이끌어 낼 수 있는 것으로 알려져 있다[54,55]. 이러한 가압처치와 결합한 저항성 운동, 유산소 운동에서 남성을 대상으로는 대부분 허벅지 둘레의 유의한 증가를 가져왔다. 선행연구결과를 참고하여 운동 유형, 운동 강도 등에 따른 남성의 허벅지 둘레의 변화를 Table 2에 요약 제시하였다.

결 론

본 종설에서는 허벅지 둘레와 대사 및 심혈관계질환의 위험요인 간 상관관계를 알아보고, 허벅지의 근육 및 지방의 체내 글루코스 및 지방대사능력에 대한 연구들을 고찰한 결과, 다음과 같은 결론을 내렸다.
허벅지 둘레를 측정하고 분석함에 있어 남성의 경우 지방과 근육량을 동시에 포함하고 있는 반면, 여자는 피하지방량을 주로 나타내는 것으로 보고되었다[16,17]. 남성의 허벅지에 있는 피하지방은 글루코스 대사에 긍정적 영향을 미쳤으며, 지질농도에 긍정적인 영향을 끼친 것으로 사료된다[16,17]. 또한 복부의 내장지방이 지질 및 글루코스 대사 이상 등의 심혈관계 질환의 위험인자와 밀접한 관련이 있는 반면에[25-27], 허벅지 지방의 경우 FFA를 흡수하여 체내 인슐린 저항성을 줄여 체내에 긍정적 결과를 가져올 수 있다는 가능성을 제시하고 있다. 일부 지방조직은 심장, 뼈, 근육, 중추신경, 신진대사에 작용하는 다수의 호르몬과 염증을 유발하는 아디포카인(adipokine)을 분비하는 것으로 보고되고 있다[56,57]. 또한 복부 내장지방조직에서 분비되는 인간에게 유해한 아디포카인의 경우 인간의 몸에 대사적 문제를 발현시키지만[58-61], 피하지방조직의 경우 아디포카인의 분비여부와 혈관질환과의 관계가 불명확한 것으로 알려져 있다[56]. 오히려 위에서 언급했듯이 피하지방조직이 인간의 대사 기전에 긍정적인 역할을 한다는 점을 고려하면[16,17], 지방 축적이 위치나 그 형태에 따라 다른 영향을 미칠 수도 있다는 가능성을 암시한다. 때문에 대퇴부에 있는 지방조직의 역할에 대한 보다 세밀한 연구가 필요할 것으로 보인다.
또한 남성의 허벅지 둘레가 골격근 및 피하지방과 양의 상관관계가 있는 반면, 여성의 허벅지 둘레는 피하지방과 양의 상관관계가 있는 것으로 보고되었고[16,17], 이러한 성별에 따른 허벅지 둘레의 구성성분의 차이를 고려한 연구가 필요할 것으로 보인다.
여성을 대상으로 수중운동을 실시한 연구집단에서 허벅지운동의 적절한 프로그램 설정이 허벅지 둘레의 향상을 가져온 것으로 판단된다[47]. 운동 형태에 따른 허벅지 둘레변화에 대한 선행연구들을 고찰한 결과 여성을 대상으로 한 대부분의 연구에서 운동을 통한 허벅지 둘레 변화의 유의미한 결과를 확인하기 어려웠다. 이는 두 가지 제한점을 가진 것으로 예측된다. 첫째, 여성을 대상으로 한 대부분의 연구들이 운동 중재 방법에 있어서 허벅지 이외에 다른 부위의 운동 개입이 많았다[44-46]. 두 번째, 위에서 언급했듯이 남성과 여성의 허벅지를 구성하는 인자들이 차이가 있다[16,17]. 여성을 대상으로 한 허벅지 둘레 향상을 위한 적절한 운동프로그램이 처치된 연구들이 현재까지 매우 미흡한 실정이다. 따라서, 향후 이를 고려한 연구들이 추가적으로 이루어져야 할 필요가 있다. 남성들의 경우 대학생, 중년남성, 엘리트 축구선수의 경우 운동 중재에 따라 허벅지 둘레의 유의한 증가를 확인할 수 있었다[48-53]. 남자 대학생을 대상으로 시행한 1회성 가압 저항성 운동을 통해 허벅지 둘레의 유의한 증가를 확인할 수 있었다[53]. 이는 운동을 통한 일시적으로 국소부위의 부종 때문일 것으로 해당 연구에서 판단하였으나, 1회성 운동 중재로 인한 허벅지 둘레의 증가는 장기간의 트레이닝 시에 허벅지 둘레 변화에 긍정적 결과를 가져올 수 있는 가능성을 암시한다. 허벅지 운동만을 집중적으로 시행한 연구에서는 모든 집단이 허벅지 둘레가 유의하게 증가했으며[51], 운동 강도에 따라 허벅지 둘레가 증가되는 정도가 차이가 있었음을 확인할 수 있었다[51]. 이는 허벅지 근력 운동이 근육 내 단백질 합성(동화 작용)을 유도하는 신호들이 증가를 통한 근육의 비대(hypertrophy)가 허벅지 둘레를 증가시킨 것으로 사료된다[62,63]. 또한 테스토스테론(Testosterone)이나 IGF-1 (Insulin like growth factor-1)과 같은 단백질 합성에 관련된 호르몬 분비[64-66]가 남녀 간 차이가 있으므로 이러한 동화 작용 호르몬이 운동을 통한 남녀 간의 허벅지 둘레 차이에 영향을 미쳤는지에 대한 후속 연구도 필요하다고 사료된다.
운동 중재를 통한 허벅지 둘레의 증가는 대사질환 및 심혈관계질환의 위험으로부터 긍정적인 결과를 가져올 것으로 판단된다. 허벅지 둘레를 구성하는 골격근과 피하지방이 심혈관계질환 및 대사질환 예방 및 개선에 긍정적인 역할을 하며, 허벅지 둘레와 심혈관계질환 간 부적 상관관계에 대한 선행연구들은 많지만[17-21,23,28], 아직까지 이를 토대로 운동 중재를 통해 직접적인 허벅지 둘레의 변화를 관찰한 연구는 매우 부족한 실정이다. 본 연구 주제와 관련된 대다수의 연구들이 허벅지 둘레의 증가 유무를 파악하기 위한 의도가 아니었다. 이는 연구결과에서 신체 구성 변화 유무를 파악하는 요소로써 허벅지 둘레가 측정되었기 때문에 향후 허벅지 둘레 증가를 위한 효과적인 운동 중재 방법에 대해 보다 세밀한 추가 연구가 필요하다.
