비만인의 체중감량을 위한 운동중재 방법에 대한 고찰

Abstract

PURPOSE

The prevalence of obesity in South Korea has been greatly increased over the past decades despite increases in one’s participation in weight reduction programs including exercise intervention. While it has been repeatedly shown that exercise has positive impacts on many health outcomes including lean body mass, fat mass, and insulin sensitivity, its effect on weight reduction is largely unclear. The purpose of this review was to summarize available data on the effect of exercise intervention on weight reduction and provide guidelines for developing effective exercise intervention, applicable to clinical practice.

METHODS

In this review, we reviewed currently available data mainly from meta-analytic studies and other available literature on the effect of therapeutic exercise intervention on weight loss.

RESULTS

Studies have generally shown that exercise intervention alone may not be effective in weight reduction due in part to insufficient exercise volume, compensatory increases in food intake, and so on.

CONCLUSIONS

It seems that exercise, especially when combined with other interventions such as diet, may enhance effectiveness of the weight loss intervention through increases in lean body mass and/or decreases in fat mass in an exercise volume dependent manner.

서 론

한국의 비만 유병률은 1998년 26.0%에서 2013년 32.5%로 지난 20년 이내에 극적으로 증가하고 있다. 여성의 비만율은 아직까지 27.5%로 유지되고 있지만, 남성의 비만율은 37.6%로 증가하였고, 특히 40대 이상 남성의 비만율은 40% 이상으로 증가하였다[1]. 이러한 비만은 대사와 심혈관계 질환의 위험을 증가시켜 건강과 관련된 의료비용을 증가시킨다. 또한, 당뇨병, 심혈관질환, 수면 무호흡증, 근골격계 통증과 개인의 일상생활 수행능력 장애 등의 발생비율이 비만한 사람들에게 높게 나타나며[2], 이러한 한국의 성인비만을 해결하기 위해 1조 7,923억 원에 달할 정도로 높은 사회경제적 비용이 발생되고 있지만 아직도 비만은 여전히 증가하고 있는 추세이다[3].
비만 예방과 체중감량을 위해 약물, 행동요법, 식이요법, 신체활동과 운동 등 다양한 중재방법들이 사용되는데, 운동의 체중감량 효과는 다른 중재요법들과 비교했을 때 매우 제한적이지만[4], 운동은 체중감소 이외의 건강과 관련하여 다른 많은 추가적인 이익을 제공한다. 규칙적인 운동은 혈중의 당조절 능력을 증진시키고 제2형 당뇨병의 진행을 늦추며, 혈압, 총 콜레스테롤, 저밀도지단백질 콜레스테롤 및 중성지방을 감소시키는 효과가 있다[5]. 또한 비만인의 삶의 질과 우울증 같은 증상을 완화시키는 효과도 있는 것으로 보고되었다[6]. 특히 상대적으로 적은 체중감소(약 5-7%)로도 임상적으로 유의한 건강상의 개선효과를 나타내는 것으로 보고되므로 운동을 통한 체중감소는 매우 중요하며[7], 체중감소 후 감량된 체중을 유지하는 데에도 운동은 중요한 역할을 한다[8].
미국스포츠의학회(American College of Sports Medicine, ACSM) [9]는 건강을 유지하고 체중감소를 촉진시키기 위해 신체활동량 혹은 운동량을 증가시키도록 권고하였으며, 규칙적인 신체활동이 체중의 증가를 예방하고, 비만과 관련된 질병의 위험을 낮추는 것으로 보고하였다[10,11]. 그러나 이와 같이 운동의 중요성과 필요성에도 불구하고 운동중재에 따른 체중감량 효과에 대해 일치되지 않는 결과들이 보고되고 있으므로 운동 프로그램의 설계 및 현장에서의 적용이 어려운 실정이다.
따라서 운동중재에 따른 선행연구들의 체중감량 효과를 운동종류, 강도, 운동량 및 식이제한병행에 따른 결과들로 정리하여, 체중감량을 위한 운동프로그램을 계획할 때 적절한 운동방법을 제시할 수 있도록 기초자료를 제공하는 것이 본 연구의 목적이다.

본 론

해외의 경우와 마찬가지로 국내에서도 운동을 통하여 체중감량에 성공하는 비율은 약 13%로 매우 낮은 것으로 보고되고 있다[1]. 그러나 운동의 체중감량 효과가 제한적이라고 할지라도, 체중의 변화가 없거나 5 kg 미만의 적은 체중감소만으로도 과체중과 비만인들의 질병과 관련된 위험요인들을 감소시키므로[12,13] 운동은 체중감량 프로그램에 포함되어야 할 중요한 요인이 된다.
선행의 여러 메타분석 연구들[14-16]을 살펴보면, 식이제한이 병행되지 않는 운동 단독중재는 체중감량의 효과가 없거나 매우 제한적인 것으로 나타났다. Murphy et al. [17]의 걷기운동을 통한 체중감량의 효과에 대한 24개의 연구를 메타 분석한 보고에서도 평균 34.9주(8-104주)의 기간 동안 걷기운동 프로그램을 중재하였을 때, 체중의 감소는 0.95 kg, 체지방률은 1.9% 감소된 것으로 나타나 걷기운동의 체중감량 효과는 매우 적은 것으로 보고하였다. 그러나 Murphy et al. [17]의 메타분석에 포함된 연구를 개별적으로 살펴보면 운동에 따른 체중 및 체지방량의 감소효과가 운동빈도 및 중재기간, 그리고 운동강도가 증가함에 따라서 효과가 크게 달라짐을 볼 수 있다(Table 1). 따라서 본 논문에서는 운동종류, 운동강도, 운동량에 따른 운동중재의 체중감량효과를 논의하고자 한다.

1. 운동종류에 따른 체중감소 효과

일반적으로 체중의 감소와 유지에 유산소 운동이 많이 권장되어 왔으나[9,18], 체중의 유지와 감소 및 체지방량 감소를 위해서는 저항운동을 실시해야 한다고 권장하는 연구[19,20]도 있으므로 운동프로그램의 적용에서 효과적인 운동선택은 아직도 불분명하다.
Willis et al. [21]은 196명의 과체중과 비만성인을 대상으로 8개월 간 중·고강도의 유산소운동, 저항운동, 또는 이 둘을 병행한 복합운동(65-80 %VO2peak와 8-12RM, 주 2-3회, 주당 134-180분)이 신체조성에 미치는 영향을 비교 연구한 결과, 체중과 체지방량의 변화는 저항운동군(0.83 kg과 0.65%)이 유산소운동군(1.76 kg과 1.01%)과 복합운동군(1.63 kg과 2.04%)에 비해 유의하게 낮은 것으로 보고하였다. Ho et al. [22]도 66명의 비만성인을 대상으로 유산소운동, 저항운동, 복합운동(60-75 %HRR와 8-12RM, 주 3회, 1회 30분)을 12주간 중재한 결과, 저항운동군(0.1 kg과 0.5%)이 유산소운동군(0.9 kg과 0.5%)과 복합운동군(1.6 kg과 1.0%)보다 낮은 체중 감소를 나타낸 것으로 보고하였다.
반면, Church et al. [23]은 262명의 비만한 제2형 당뇨환자를 대상으로 중·고강도의 운동(50-80 %VO2max와 10-12RM, 주 2-3회, 주당 150분)을 9개월 간 운동중재한 후 체중과 체지방량의 감소량이 복합운동군(1.5 kg과 1.7 kg)이 저항운동군(0.3 kg과 1.4 kg)과 유산소운동군(0.8 kg과 0.6 kg)보다 높았으나 저항운동군과 유산소운동군 간의 유의한 차이는 없다고 하였다. Yavari et al. [24]도 80명의 비만한 제2형 당뇨병 환자를 대상으로 주 2-3회의 운동을 실시한 후 체중과 체지방량의 감소가 유산소운동군(0.7 kg과 1.00%)이 저항운동군(1.2 kg과 2.2%)보다 오히려 낮은 것으로 보고하였다.
선행연구들이 일치되지 않는 결과들을 보고하고 있으나 전반적으로는 유산소운동 중재가 저항운동 중재에 비해 좀 더 효과적인 것으로 보인다. 그러나 운동종류에 따른 체중감량 효과는 아직 명확하지 않은 듯 하며, 운동종류에 따라 신체조성 변화에 미치는 영향의 차이가 있는 것으로 제시되었다. 이와 관련하여 Slentz et al. [25]은 유산소 운동은 체지방량을 감소시키고 저항운동은 제지방량을 증가시켜 체지방률을 감소시킨다고 제시하였다. Willis et al. [21]의 연구결과를 보아도 저항운동군은 제지방량이 1.09 kg 증가하였고, 유산소 운동군은 0.10 kg이 감소한 것으로 나타나 이러한 주장을 뒷받침하고 있다(Table 2). 즉 유산소운동은 운동 시 지방산화와 에너지소비량을 증가시켜 체지방량을 감소[26]시키는 반면, 저항운동은 제지방량을 증가시켜 안정 시 대사량을 높여 부적(negative) 에너지 균형을 유도하게 됨으로써 체중을 감소시키는 것으로 제시되었다[27,28].
그러나 실질적으로 유산소운동과 저항운동의 효과를 비교하기에는 어려움이 있다. 그 대표적인 이유는 유산소운동과 저항운동 간 운동량을 동일하게 처방하기가 어렵기 때문에 대부분의 연구들이 유산소운동과 저항운동의 운동량을 동일하게 실시하지 않았다[20,29]. 또한 복합운동의 경우, 유산소운동 또는 저항운동만을 수행한 경우에 비해서 체중 및 체지방량의 감소가 더 많은 것으로 나타나지만, 이러한 효과는 아마도 더 많은 운동량에 기인한 것으로 보인다[21]. 또한, 기전적인 원인은 불확실하지만 운동 시 소모되는 에너지 소비량을 고려하더라도 유산소운동이나 저항운동만을 단독 중재하는 것보다는 병행하는 것이 효과가 있다는 연구결과도 보고되었으므로[22,23], 보다 명확한 결론을 도출하기 위해서는 추가적인 연구와 분석이 필요하다.
이상의 내용을 정리해보면, 유산소운동은 체지방량의 감소를 통해서, 그리고 저항운동은 제지방량의 증가를 통해서 체중 감소를 유발하는 것으로 보고되지만, 운동종류에 따른 차이는 운동량이 일치되지 않는 경우가 많기 때문에 아직까지 불분명하다. 반면, 유산소와 저항의 복합운동이 유산소와 저항운동의 단독 중재보다 각각의 운동종류에 따른 효과를 기대할 수 있으므로 운동적용에는 더 용이할 것이다.

2. 운동강도에 따른 체중감소 효과

체중 감량에 대한 운동중재 효과를 비교한 연구들은 결과의 차이를 운동강도의 차이로 제시하기도 한다. 이는 운동강도가 단위시간 당 사용되는 에너지 소비량과 운동 시 사용되는 연료(탄수화물과 지방)의 선택과 사용량에 영향을 주기 때문이다[30]. 일반적으로 운동강도가 증가함에 따라 수축하는 근육은 에너지원으로 탄수화물 산화(혈당 및 근육글리코겐)에 의존하며, 특히 고강도 운동에서는 근육글리코겐을 주된 연료로 사용한다. 반면 지방의 산화에 대한 의존도는 저강도에서는 혈중유리지방산이 에너지원으로서 주된 역할을 하지만, 중강도로 증가하면서 총 지방산화(근육내 중성지방과 혈중유리지방산)는 증가되는 반면 혈중유리지방산의 역할이 감소되고, 고강도 운동에서는 상대적·절대적 지방산화량이 탄수화물 산화에 비해서 유의하게 감소한다[31,32].
이와 같이 운동강도에 따라 동원되는 에너지원이 다르기 때문에 운동강도에 따른 체중 감소 및 체지방량의 감소 효과는 차이가 있을 것으로 보이지만[33,34], 운동강도는 운동 중, 그리고 운동 후 에너지소비량(즉, 운동 후 초과산소소비량)의 증가에 직접적으로 영향을 주기 때문에, 전체적인 에너지 소비량 측면에서는 고강도 운동이 체중 및 체지방의 감량에 더욱 효과적일 수도 있음을 간과해서는 안 된다[35,36]. 이에 운동강도에 따른 운동중재효과에 대해서 더 논의해보고자 한다.
운동중재 후 약 10-20 kg 정도까지 체중이 큰 감소를 나타낸 연구에서는 주 6-7일 정도의 운동훈련을 고강도(≥ 65 %VO2max)에서 최소 30분에서 2시간까지 실시한 것으로 나타났다. 극단적인 경우지만 Lee et al. [37]은 비만 남성들을 대상으로 5개월간 주당 29시간의 고강도 군사훈련을 실시한 후 약 12.5 kg의 체중 감소가 나타났다고 보고하였다. Van Loan et al. [38]도 14주간 주 3-7회, 65-85 %VO2max에서 1회 30-120분의 유산소 운동을 실시한 후 4.3 kg의 체중감소가 나타났다고 하였다. Keim et al. [39]도 12주간 주 6회, 65-85 %VO2max에서 1회 35-45분의 걷기 운동을 실시한 후 약 5.6 kg의 체중감소가 나타난 것으로 보고하여 고강도의 운동훈련이 체중감량 효과가 있는 것으로 보고하였다. 또한, Grediagin et al. [40]도 고강도 운동이 중강도 유산소 운동에 비하여 더 많은 체중감소를 유도하고, 비 훈련인에게는 만일 같은 양의 체지방량이 감소하더라도 더 많은 제지방량을 증가시키는 것으로 보고하였다.
그러나 모든 연구들이 이러한 결과를 보고하는 것은 아니다. 고강도 운동훈련에도 불구하고 운동중재 후 체중의 감소가 크지 않다는 연구들도 보고되고 있는데, Church et al. [23]은 4개월간 주 3-5회, 60-80 %VO2max에서 1회 30분 이상의 유산소 운동을 실시한 후 체중감소량이 0.4 kg만 나타났다고 보고하였다. Kraus et al. [41]도 운동강도와 운동량으로 집단을 분류하여 비교한 결과 저운동량/중강도군(주당 176분, 40-55 %VO2max)의 체중감소량은 0.6 kg, 저운동량/고강도군(주당 117분, 65-80 %VO2max)의 체중감소량은 0.2 kg, 고운동량/고강도군(주당 171분, 65-80 %VO2max)의 체중감소량은 1.5 kg으로 나타나 운동강도나 운동량과 상관없이 운동중재만으로는 체중의 감소가 제한적인 것으로 보고하였다. 이와 같이 고강도(≥ 60 %VO2max 또는 65-85 %HRR)에서 운동을 실시한 후 대조군에 비하여 약 2-4 kg 정도 더 감소되었다는 연구보고도 있지만[42-46], 주당 운동량을 동일하게 처방하고 운동강도만 달리한 경우에는 집단 간 체중감소량의 차이가 없다는 보고[47]와 차이가 있다는 보고[48] 등 일치되지 않는 결과들이 보고되고 있다(Table 3).
이러한 운동강도에 따른 체중감소 효과의 차이는 운동중재 기간동안 실질적으로 수행된 운동량과 강도가 처방된 것과는 차이가 있음으로 인해 나타나는 듯하다. 예를 들면, 운동지도자(personal trainer)와 운동을 같이 실시한 경우와 피험자가 혼자 운동을 한 경우에 차이가 나타날 수 있다고 제시되었다[49]. 또한, 운동강도에 따른 효과 차이를 분석하기에도 어려움이 있는데, 이는 운동중재기간 동안 운동강도를 고정하지 않고 점진적으로 증가시키는 방법을 사용한 연구도 있기 때문이다. 즉 초기의 운동강도는 50-60 %VO2max에서 시작되었으나 운동훈련의 종료 시점에서는 70-85 %VO2max로 점증적으로 운동강도를 증가시킨 연구들이 많아[21,23,45,46] 운동강도에 따른 체중 감소 효과를 비교하기에는 어려움이 있다.
또한 운동은 비만인들의 체중감량뿐만 아니라 체력요인을 증가시켜 비만으로 인해 발생할 수 있는 다양한 질병의 위험요인을 감소시키는 효과가 있다. 고강도 운동은 체중감량 이외의 다양한 건강요인들에도 긍정적인 효과를 유발하는 것으로도 보고되고 있다. 예를 들어, 심폐체력의 지표인 최대산소섭취량의 향상은 운동강도에 영향을 받으며, 다양한 비만관련질병 위험요인, 유병률 및 사망률과 부적인 상관관계가 있는 것으로 알려져 있다. Lee et al. [50]은 3,148명을 대상으로 6년간 심폐체력과 고혈압, 대사증후군, 이상지질혈증의 발병률을 조사한 결과 심폐체력이 유지되거나 증가한 경우에는 체지방률이 증가하여도 고혈압(1.2배), 대사증후군(1.6배), 이상지질혈증(1.3배)의 발병률이 크게 증가하지 않는 데 비해, 심폐체력이 감소한 경우에는 체지방률 증가에 따라 고혈압(1.7배), 대사증후군(2.8배), 이상지질혈증(1.9배)의 발병률이 더 크게 증가하는 것으로 보고하였다. 따라서 심폐체력은 체지방률과 독립적으로 질병의 발병률에 중요한 역할을 하는 것으로 보이며, 심폐체력을 증진시키기 위해 중·고강도의 운동이 권장된다[9,51].
이상의 내용을 정리하면 운동 중에는 저강도 운동이 상대적으로 지방산화를 증가시키고 고강도 운동이 탄수화물 산화를 증가시키지만, 전반적으로 중·고강도의 운동이 제지방량을 유지하거나 증가시키고 체중을 감소시키는 데 더욱 효과적인 것으로 보고되고 있으며[17,47], 운동 시 에너지 소비량과 운동 후 초과산소섭취량 측면 및 심폐 체력의 증가면에서도 중·고강도 운동이 총 에너지 소비량이 높으므로 체중감량에는 더 효과적일 수 있을 것으로 사료된다.

3. 운동량에 따른 체중감소 효과

체중감소와 감량된 체중을 유지하기 위한 충분한 운동량은 얼마나 될 것인가? 앞에서 논의된 운동에 따른 체중감량이 적게 나타나는 이유는 아마도 메타분석에 포함된 연구들의 대부분이 주당 운동량이 상대적으로 적게 처방되었기 때문으로 보인다[16]. ACSM은 건강상 유익한 효과를 위해서 주당 최소 1,000 kcal에서 2,000 kcal까지 에너지 소비량 증가를 권고하고 있으나[9], 체중 감소를 위해서는 최소 주당 150분이나 2,500 kcal의 더 많은 에너지 소비가 필요한 것으로 제시되었는데, 이러한 에너지소비량은 80 kg인 사람에게 주당 70,000보의 보행 수에 해당된다. 또한, 식이제한 없이 운동만으로 더 많은 체중감소를 위해서는 주당 200-300분이나 3,500 kcal (주당 90,000보)의 운동을 실시하는 것이 더 효과적인 것으로 제시되었다[52].
Jackicic et al. [53]은 주당 신체활동량이 150분 이하인 집단의 체중감소량은 3.5 kg, 주당 150-199분 신체활동량을 가진 집단의 체중감소량은 8.5 kg이고, 주당 200분 이상의 신체활동군의 체중감소량은 13.1 kg으로 신체활동량이 많을수록 더 많은 체중감소를 나타내는 것으로 보고하였다. Wing et al. [54]도 주당 신체활동량이 168-616 kcal인 집단은 저활동군, 700-1,200 kcal인 집단은 중활동군, 1,372-4,116 kcal인 집단은 고활동군으로 분류하여 1년 후 체중감소량을 비교한 결과 저활동군은 2.3 kg, 중활동군은 5.9 kg, 고활동군은 9.1 kg 감소한 것으로 보고하였다. 또한 Jackicic et al. [55]의 또 다른 연구에서도 운동 중재 후 12개월 동안 관찰한 결과 신체활동량이 주당 150분 이하인 신체활동군의 체중 유지율은 4.7%, 주당 150-199분인 신체활동군의 체중 유지율은 9.5%, 주당 200분 이상인 신체활동군의 체중 유지율은 13.6%로 신체활동량이 많을수록 감소된 체중을 유지하는 데 더욱 효과적인 것으로 나타났다(Table 4).
Ross et al. [56]도 12주간 1회 700 kcal를 소비하는 유산소 운동을 주당 5일 실시한 후 8%의 체중감량이 나타났다고 하였으며, Donnelly et al. [19]은 16주간 주당 2,000 kcal 이상을 소비하는 유산소 운동 후 5.3%의 체중감소가 나타났다고 보고하였다. Hadjolova et al. [57]도 32명의 비만남성을 대상으로 45일간 고강도의 군사훈련을 1일 10시간(3,600-3,700 kcal)을 실시한 결과 12.5 kg이 감소한 것으로 보고하였다. 따라서 운동을 통해서 체중을 효과적으로 감소시키기 위해서는 1일 500-1,000 kcal의 에너지 소비가 있어야 한다고 제시하였다[16].
그러나 Rosenkilde et al. [58]은 운동량(즉, 에너지소비량)과 체중감소량 간에 반드시 용량반응(dose-response)을 나타내는 것은 아니며, 운동의 체중감량에 대한 역치(threshold)에는 성차, 개인차, 그리고 운동형태 등 많은 요인들이 영향을 준다고 하였다. 예를 들어, 운동으로 인한 에너지 소비량이 7,700 kcal일 때 체중이 1 kg 감소되는데, 그 중 체지방량 감소가 70%, 제지방량 감소가 30% 기여하게 된다. 따라서 제지방량의 감소로 인한 기초대사량 감소와 같은 보상작용이 발생할 수 있음을 유의해야 한다[59,60].
이상의 내용을 정리하면, 체중의 감소와 감량된 체중을 유지하기 위해서는 높은 운동량(신체활동량)을 유지해야 한다. 이를 위해서는 주당 200분 이상이나 2,500 kcal 이상의 에너지를 소비하는 운동을 권장(예, 주 6회 운동을 할 경우, 중강도 이상에서 30분 정도)하는 것이 바람직할 것으로 사료된다. 그 이상의 운동은 운동량(시간)에 비례하여 체중감량 효과가 나타나는 것은 아니지만, 건강과 관련된 추가적인 효과가 있는 것으로 보인다[58,61]. 또한 유산소 운동중재 후 1년을 추적 조사한 결과에 따르면 대조군의 경우 2.9 kg 체중이 재증가한 것에 비하여 운동을 규칙적으로 실시한 집단은 체중의 변화가 0.7 kg으로 매우 적게 나타나므로[19] 운동은 체중의 지속적인 관리에서 중요하게 고려되어야 할 것이다.

4. 식이제한을 병행한 운동중재에 따른 체중감소 효과

체중감량을 위한 운동 프로그램의 계획에서 운동관리만큼 중요하게 고려해야 되는 문제는 에너지균형 조절이다. 쉽게 말해서 체중의 변화는 인체 내로 들어온 에너지와 다양한 경로(예를 들면, 기초대사량과 신체활동 등)를 통해 사용되는 에너지 간의 균형이기 때문이다. 따라서 운동중재를 통한 효과적인 체중감량을 이루고자 한다면, 음식섭취량의 조절 및 통제가 항상 고려되어야 할 것이다.
Kelley et al. [62]의 연구에서 식이제한집단, 식이제한과 운동을 병행한 집단, 운동만 실시한 집단의 6개 연구를 메타분석한 결과 운동만 실시한 집단에서는 0.8 kg으로 체중의 감소가 별로 없었고, 식이제한과 식이제한 및 운동을 병행한 집단에서의 체중감량(각 3.0과 3.5 kg)과 체지방률(각 5.5와 6.1%)의 변화가 더 높은 것으로 나타났다. 그러나 이 메타분석에서는 집단별 식이섭취량의 변화도 같이 조사하였는데, 식이제한집단과 식이제한과 운동을 병행한 집단에서는 에너지 섭취량이 각각 246 kcal와 181 kcal가 감소되었지만 운동집단에서는 오히려 에너지 섭취량이 60 kcal 증가된 것으로 나타나 운동을 한다고 해도 운동 전보다 더 많은 음식을 섭취할 경우, 즉 식이조절을 하지 못해서 에너지 균형을 맞추지 못하게 되면 체중감량의 효과를 기대할 수 없을 것이다.
또한, 운동중재와 식이제한의 병행은 추가적인 부적 에너지 균형을 유발하여 체중의 감소를 더 빠르게 할 것이며 체중감소 효과를 증가시킬 것으로 기대하였다[4,63]. Catenacci et al. [16]은 17개의 연구를 메타분석한 결과에서도 식이제한만을 실시한 것보다 운동을 병행한 집단의 체중감소가 더 크며, 평균 1.5 kg 정도가 더 감소되었다고 보고하였다. Gordon et al. [64]도 운동과 식이를 병행한 경우(7.1 kg) 식이제한만을 실시한 경우(5.8 kg)에 비해 효과가 증가하는 것으로 보고하였으며, Keim et al. [39]도 식이제한에 주 6회 걷기운동을 실시한 집단의 체중 감소가 13.1 kg으로 운동만을 실시한 집단의 5.6 kg보다 더 큰 감소가 있었던 것으로 보고하였다.
그러나 Miller et al. [65]은 493개의 연구를 메타 분석한 결과[식이제한연구(224개), 유산소운동연구(76개), 식이제한과 운동을 병행한 연구(199개)], 식이제한과 운동을 병행한 집단은 11 kg (11.7%) 감소하여 유산소 운동군보다 2.9 kg 더 감소한 것으로 나타났으나, 식이제한군의 체중감소량이 10.7 kg (10.5%)으로 집단 간 체중감소량의 차이가 유의하지 않았다고 하였다. Susanne et al. [14]의 메타분석 연구에서도 15개의 연구 중 단지 4개의 연구만이 식이제한에 추가적인 운동의 병행이 체중감소를 더 증가시키는 것으로 보고하였고, Katzel et al. [66]은 미국심장협회에서 권고하는 식이제한에 주 3회 45분의 유산소 운동을 9개월간 실시한 결과 중재 간 체중감소의 차이가 없는 것으로 보고하였다. Lefevre et al. [67]도 6개월 간 식이제한과 운동을 병행하였을 때 식이제한군과 운동을 병행한 집단 모두 6개월 동안 약 10%의 체중 감소와 약 25%의 복부지방 감소가 나타났으나 두 집단 간의 차이는 없는 것으로 보고하였고, Morencos et al. [68]도 22주간 식이제한과 유산소 및 저항운동을 실시한 결과 저항운동집단이 9%, 유산소운동집단이 9.4%, 저항운동과 유산소 병행집단이 10.5%, 식이제한 집단이 7.6%로 나타나 유의한 차이가 없는 것으로 보고하였다.
이와 같이 식이제한과 운동의 병행효과에 대한 일치되지 않는 결과에 대해 Susanne et al. [14]은 주당 운동빈도 차이가 체중감소에 영향을 주었을 것으로 제시하였다. 즉 더 많은 운동빈도는 에너지 소비량을 증가시키므로 더 빠른 체중감소를 유도할 것이라는 것이다. 그러나 체중의 추가적인 감소가 나타난 3개의 연구에서 운동횟수가 주당 5-6일의 빈도였고, 체중의 추가적인 감소가 없었던 7개의 연구에서는 운동횟수가 주당 3-7일의 빈도로 실시된 점을 고려한다면 주 3회 이상의 운동횟수에서는 빈도에 따른 차이는 없는 것으로 보인다[4].
또 다른 이유로 Catenacci et al. [16]은 운동중재에 의한 에너지 소비량(주당 1,000-1,500 kcal)이 식이제한(1일 500-1,000 kcal 또는 주당 3,500-7,000 kcal)에 비해 유의하게 낮기 때문인 것으로 제시하였다. 즉, 대부분 경우 운동 시간은 주당 60-240분까지였으며, 17개의 연구 중 ACSM [9]에서 권고하는 체중감량을 위한 운동시간, 즉 주당 200분 이상의 시간이 처방된 연구는 단지 4개로 식이제한량에 비하여 운동량이 현저히 적기 때문이라고 제시하였다.
또한, Johns et al. [69]의 3-6개월 동안 식이제한과 운동을 병행한 6개의 연구에서는 운동병행이 식이제한 단독중재에 비해 적은 체중감소량(0.62 kg)을 유발했으나, 좀 더 장기간인 12개월 동안 식이제한과 운동을 병행한 7개의 연구에서는 식이제한의 단독중재보다 운동을 병행한 경우의 체중 감소량이 1.72 kg 더 큰 것으로 나타났다고 보고하였다. 이것은 비록 식이제한 시 운동의 병행이 초기에는 추가적인 체중감소 효과가 적지만, 감소된 체중을 유지하는 데 더 도움이 되기 때문인 것으로 제시하였다[16].
운동과 식이제한의 병행중재에 따른 체중감량 효과와 관련하여 알려진 생리학적 기전을 살펴보면 다음과 같다. 식이제한은 에너지 이용과 저장 및 식욕조절과 같은 신체 생화학적 기능의 변화와 보상을 유도하는데[70,71], 식이제한에 따른 체중감소는 안정 시와 1일 총 에너지 소비량[72,73], 지방 산화능력[74], 갑상선 호르몬 수준[75]을 감소시키며, 코르티솔 수준을 증가시켜[76] 오히려 에너지 저장능력을 증가시키게 된다. 또한 식이제한은 위장의 식욕촉진과 관련된 그렐린 호르몬의 수준을 증가시키고[77], 식욕억제와 관련된 렙틴[78]과 YY 펩타이드 수준을 감소시키며[79,80], 교감신경계의 변화를 유도하여[81] 오히려 에너지 섭취량을 증가시키게 된다. 따라서 이러한 호르몬과 에너지 소비량의 변화들은 장기간 동안 식이제한에 따른 체중감소를 유지하기 어렵게 만드는 요인이 된다.
반면 운동을 병행할 경우, 에너지 소비량을 추가적으로 증가시킬 뿐만 아니라[82] 식욕조절 호르몬의 변화[83,84] 및 식이섭취와 관련된 뇌신경반응의 조절[85]을 통해 장기간 동안의 감소된 체중을 유지하는데 도움이 되는 것으로 알려져 있다. 또한, Hunter et al. [86]은 1일 800 kcal를 섭취하는 식이제한 동안 주 3회, 1회 40분의 유산소 운동을 80 %HRmax에서 실시한 후 1년간 추적조사를 실시한 결과, 운동을 지속한 집단의 체중증가는 4.9%로 나타났지만 그렇지 않은 집단의 체중증가는 9.9%로 더 많은 증가를 나타냈다고 보고하였다.
또한 식이제한을 하는 동안에 운동을 병행하는 것이 제지방량을 유지하거나 증가시키고 체지방량의 더 많은 감소를 유도하며[87-89], 중재기간 후 감소된 체중을 유지하는 데 도움을 주는 것으로 보고되므로[54], 체중감량 프로그램의 계획 시 체중감소량에 상관없이 운동을 중요한 요인으로 고려해야 할 것이다.
이상의 내용을 정리해보면, 식이제한과 운동을 병행하여도 초기 체중감소에는 추가적 효과가 적지만, 장기간 동안의 체중감소와 감소된 체중을 유지하는 데는 도움이 되는 것으로 나타났다. 또한 운동 병행의 추가적인 체중감량 효과를 나타내기 위해서는 식이제한량 만큼 체중에 변화를 줄 수 있는 충분한 운동량을 처방하여야 할 것이다.

결 론

본 연구는 체중감량 프로그램에서 운동효과에 대한 선행연구들의 결과들을 운동종류, 강도, 운동량에 따라 분석한 결과 다음과 같은 결론을 도출하였다. 체중감량 프로그램에서 운동중재의 효과를 위해서는 주 3회 이상의 중·고강도 운동(≥ 60 %VO2max)을 주당 200분 이상, 또는 2,500 kcal 이상을 소비하는 유산소와 저항운동을 실시해야 된다. 또한, 운동만을 실시한 경우 단독 효과는 다른 중재방법들에 비하여 적은 것으로 나타났으나 운동을 지속하는 경우 감량된 체중을 유지하는 데 도움이 되고, 심폐체력수준의 향상은 비만과 관련된 질환의 발생위험을 줄이는 것으로 보고되므로 지속적인 중·고강도의 운동이 비만인의 체력수준에 맞게 적절히 적용되어야 할 것이다.
