Abstracts

Purpose

Proprotein Convertase Subtilisin/Kexin type 9 (PCSK9) is a pivotal regulator of low lipoprotein-cholesterol (LDL-C) and LDL receptor (LDLR) metabolism, and the interest in PCSK9 has increased in cardiovascular diseases. Exercise reduces blood LDL-C via PCSK9-LDLR pathway in the liver and the vasculature. However, the mechanism of exercise-induced inhibition of PCSK is unclear. The aim of this review is to describe the role of exercise on PCSK9-LDLR axis in cardiovascular diseases.

Methods

This study review 34 previous studies focusing on the effect of exercise on PCSK9 in the human and animal.

Results

The effects of exercise and lifestyle intervention on hepatic and circulating PCSK9 are controversial. However, exercise con-sistently increases hepatic LDLR, and inhibits atherosclerosis via suppression of PCSK9 and LOX-1 in atherosclerotic region.

Conclusions

Even though experimental data are still very limited, exercise training can improves blood LDL-C via inhibition of PCSK9 and enhancement of LDLR in liver and vasculature. The study of exercise on PCSK9 are urgently needed.

서 론

심혈관질환의 주된 원인 중 하나인 동맥경화는 혈관벽에 과도한 지질의 축척으로 인해 발생되는 염증반응이다. 지난 수십년 간 많은 실험적 연구들은 혈중 콜레스테롤, 특히 저밀도콜레스테롤(Low density Lipoprotein-Cholesterol, LDL-C)이 동맥경화의 위험성과 직접적으로 관련되어 있다는 것을 보여주고 있다[13].

혈중 콜레스테롤을 낮추는 것이 심혈관질환의 위험성을 감소시킬 수 있기때문에, 현재 미국이나 유럽에서는 스타틴(statin) 처치에 의한 LDL-C의 감소가 주요 치료전략으로 강조되고 있다. 그러나 스타틴에 대한 내성이나 2형 당뇨병의 발병 등이 발견되면서[4,5], 새로운 치료기법이 모색되었다. 바로 Proprotein Convertase Subtilisin/Kexin type 9 (PCSK9)의 발견이다[6,7]. PCSK9은 간에서 LDL수용기(LDL receptor, LDLR)와 반응하여 혈중 LDL 분자와 LDLR의 결합을 방해하여 혈중 LDL-C 농도를 증가시키는 것으로 알려져 있다[79]. 따라서 최근 임상실험에서 PCSK9 억제제인 단일 PCSK9항체(Monoclonal antibody)가 혈중 LDL-C농도를 65% 정도 줄이는 효과가 있어 중증 고지혈증의 치료제로 이용되고 있다[10].

그러나 스타틴은 PCSK9 농도를 오히려 증가시킬 수 있다는 연구결과가 보고되면서[11,12] 비약물요법의 중요성이 증대되고 있다. 최근 운동이 혈중 LDL-C를 줄일 수 있는 기전 중 하나인 PCSK9-LDLR 복합체에 관한 연구들이 보고되고 있다[1315]. 따라서 이 연구에서는 동맥경화의 주범인 LDL-C의 억제 기전을 PCSK9을 중심으로 설명하고, 운동에 의한 이의 억제 기전에 관한 선행연구를 고찰하고자 한다.

PCSK9과 LDLR

혈중콜레스테롤은 약 70%가 LDL에 의해 운반되고, 관상동맥질환의 주된 위험요인으로 잘 알려져 있다. 혈중 LDL은 간세포 표면에 있는 LDLR와 결합하여 LDLR-매개 세포내유입(LDLR-mediated endocytosis)을 통해 세포내로 들어간다. 즉 LDLR는 간에서 혈중에 있는 LDL 을 제거하는 주된 단백질이다[1,16].

그러나 간세포의 LDLR과 혈중 LDL-C의 결합을 방해하는 것이 PCSK9이다. PCSK9은 Abifadel et al. [6]에 의해 보고되면서 최근 10년간 심혈관질환을 억제하기 위한 약물 개발분야에서 가장 크게 부각되었다. PCSK9은 성인의 경우 주로 간에서 생성되고 분비되며 주로 소장, 신장, 그리고 뇌에서 발현된다[17,18]. 이 단백은 간에서 혈액으로 분비되어 세포표면에 있는 LDLR의 EGF-A 도메인(Epidermal growth factor-like repeat homology domain)과 결합하여 PCSK9-LDL 복합체를 이루고, 세포내로 유입되어 리소좀에서 제거되거나 다시 세포표면으로 이동하여 재활용(recycling)된다[19,20]. 이 과정에서 LDLR도 동일하게 재활용되거나 제거되기 때문에 LDLR가 감소된다[16].

그러나 고콜레스테롤 상태에서는 간에서 생성된 PCSK9이 혈중으로 다량 분비되고, 분비된 PCSK9이 혈중의 LDL-C와 LDLR의 결합을 저해함으로써 혈중의 LDL-C 농도가 증가된다[16,21]. 또한 이러한 조절은 Sterol regulatory element binding protein-2 (SREBP-2) 단백에 의해 조절되는 것으로 알려져 있다[18]. 즉 SREBP-2는 간조직에서 LDLR 발현을 활성화시켜 간조직으로의 LDL-C 유입을 증가시키기도 하지만 PCSK9을 활성화시켜 오히려 LDLR의 분해를 촉진함으로써 콜레스테롤의 항상성을 유지하게 한다[20].

운동이 PCSK9과 LDLR에 미치는 효과

PCSK9은 간의 표면에 있는 LDLR와 먼저 반응하여 LDLR과 LDL-C 의 결합을 방해함으로써 혈중 LDL-C를 축적시킨다. 고콜레스테롤 치료를 위한 비약물요법 중 하나가 운동이다. 현재까지 운동이 PCSK9에 미치는 효과에 관한 연구들이 Table 1에 제시되어 있다. 최근 연구들은 인간대상연구에서 운동과 같은 생활방식의 개입(lifestyle interventions)이나 동물연구에서 유산소운동에 의한 PCSK9의 감소[2224]와 LDLR의 증가[13,2527]를 보고하고 있다.

Effects of exercise on PCSK9 level

그러나 PCSK9에 미치는 운동의 효과는 매우 상이하다. Kamani et al. [23]은 건강인을 대상으로 일터에서 3개월간의 엘리베이터 대신 계단운동이 혈중 PCSK9과 LDL-C 농도의 감소를 보고하고 있고, Arsenault et al. [22]도 비만인을 대상으로 1년간 생활방식의 개선(운동)이 혈중 PCSK9을 감소시킬 뿐만 아니라 PCSK9과 LDL-C (small parti-cles)의 양적 상관관계를 보고하고 있다. 뿐만 아니라 Farahnak et al. [24]은 난소제거모델의 간조직에서 mPCSK9의 감소와 혈중 PCSK9의 감소를 보고하고 있다.

이와는 반대로, Boyer et al. [28]은 관상동맥질환 환자를 대상으로 운동에 의해 혈중 PCSK9 농도가 증가하고 LDL-C 농도는 변화가 없다고 밝히고 있고, Sponder et al. [29]은 인간을 대상으로 운동이 혈중 PCSK9 농도를 증가시키지만 LDL-C는 감소시킨다고 보고하고 있다. 뿐만 아니라 동물모델에서도 유산소운동에 의해 간조직의 PCSK9발현이 증가[26]하거나 변화가 없다[13,30]고 보고되고 있다.

왜 운동이 PCSK9에 미치는 효과가 일관성이 없는 것인가? 건강인이나 특정질환과 같은 실험대상의 차이일 수도 있겠으나 현재까지 이에 대한 기전은 불분명하다. 하나의 가능성은 PCSK9 유전자 기능에 따라 다르게 나타날 수 있다. 즉 PCSK9의 기능획득 돌연변이(Gain-of-function mutations)는 혈중의 LDL-C 수준을 증가시켜 동맥경화를 유도할 수 있다[6,31]. 이와 반대로 PCSK9의 기능손실 돌연변이(Loss-of-function mutations)는 낮은 수준의 LDL-C 농도와 관상동맥질환의 감소를 유도하는 것으로 보고되고 있다[17,18]. Gu and Zhang [32]은 총설에서 PCSK9 (Y142X와 C679X)에서 두가지의 이질접합체가 있는 아프리카계 미국인은 혈장 LCL-C농도가 30–40% 감소함을 보고하고 있다. 그러나 PCSK9의 유전자변이에 의한 운동의 효과는 아직 보고된 바 없어 이에 대한 연구가 필요하다.

흥미롭게도 운동이 혈중 LDL-C와 결합하는 간조직의 LDLR발현을 증가시킨다는 보고들[13,2527]은 일관성이 있는 것 같다. 이러한 결과는 운동에 의한 혈중 LDL-C의 감소가 간조직의 LDLR를 통해 나타난다는 강력한 증거이다.

또한 PCSK9은 주로 간에서 생성되고 분비되지만 소장, 신장과 뇌에서 발현된다[17,18]. 흥미롭게도 PCSK9은 인간의 동맥경화반에서도 발현되고 PCSK9의 활성이 대식세포의 LDLR 발현을 줄이는 것으로 보고되고 있다[33]. 이러한 결과는 PCSK9이 혈관에서 직접적으로 동맥경화를 유발할 가능성이 있다는 것을 의미한다. 최근 Li et al. [13]은 동맥경화모델에서 유산소운동이 간조직에서 PCSK9의 변화는 확인할 수 없었으나 LDLR를 증가시키고, 동맥경화반에서 PCSK9의 감소와 산화 LDL의 수용기인 LOX-1 (Lectin-like oxidized LDL receptor 1)의 감소를 보고하였다. 이러한 결과는 운동이 간조직에서 LDLR를 증가시키고 혈관에서 PCSK9의 감소를 통한 혈중 LDL-C의 혈관내 유입을 억제하여 동맥경화가 개선되었을 가능성이 있다. 그러나 동맥경화와 같은 심혈관질환에서 PCSK9과 LDLR에 관한 운동기전은 불분명하다. 이에 대한 보다 많은 연구가 필요하다.

결 론

PCSK9은 간조직의 LDLR의 분해를 증가시켜 혈중 LDL-C의 증가를 초래한다. 이러한 결과는동맥경화와 같은 심혈관질환의 주된 위험요인이다. 고콜레스테롤 치료를 위한 비약물요법 중 하나가 운동이다. 운동이 PCSK9에 미치는 효과에 관한 실험적 연구들은 운동이 간조직에서 LDLR의 증가와 PCSK9발현의 감소를 통해 혈중 LDL-C를 감소시키고, 제한적 연구이지만 혈관조직에서도 PCSK9의 감소와 LOX-1의 감소를 통해 동맥경화부위로의 LDL-C의 유입을 억제할 수 있다는 것을 알 수 있었다. 그러나 운동에 의한 PCSK9의 억제효과는 일관되게 관찰되지 않았다. 이는 실험대상의 차이때문일수도 있겠으나 이에 대한 기전은 불분명하다. 그러나 PCSK9 유전자 변이의 차이에 따라 운동의 효과가 다르게 나타날 가능성은 있다. 향후 운동이 PCSK9 유전자 변이를 유발하는지, 그리고 이에 대한 변화로 인해 LDL-C 농도가 변화되는지에 관한 기전연구가 보다 필요할 것으로 생각된다.
