Trans Abstract

Purpose

The purpose of this study was to investigate the effects of short-term endurance exercise and lithium treatment on fasting blood glucose in rats with type 1 diabetes (T1DM).

Methods

Ten-week-old male C57BL/6J mice were induced with T1DM using streptozotocin (STZ) injection. They were then randomly assigned to five groups and treated for a total of 4 weeks. For lithium treatment, 10 mg/kg LiCl was orally administered once a day, 5 days a week, and endurance exercise was performed at 21 m/minutes for 60 minutes, 5 days a week. After four weeks of treatment, blood and the plantaris muscle were extracted under anesthesia, and blood glucose and biomolecular factors were analyzed.

Results

4 weeks of moderate-intensity endurance exercise significantly reduced fasting blood glucose in T1DM mice, increased the expression of GLUT4 in skeletal muscle, and significantly decreased the expression of dynamin, a GLUT4 endocytosis-related factor. Endurance exercise significantly increased mitochondrial biosynthesis in the skeletal muscles and significantly reduced the expression of the MAPK family. However, no biochemical or physiological changes were associated with lithium alone or in combination.

Conclusions

4 weeks of moderate-intensity endurance exercise improves fasting blood sugar levels through positive changes in glycemic control factors in the skeletal muscle. However, lithium treatment has no resultant effect. These results are thought to have been influenced by the decrease in the expression of the MAPK family and the increase in mitochondrial biosynthesis by endurance exercise for 4 weeks. However, further research is required to establish a clear causal relationship.

서 론

제1형 당뇨병(Type I Diabetes Mellitus, T1DM)은 자가면역기전에 의해 췌장의 베타세포가 파괴되어 인슐린이 분비되지 않음으로 발생하는 것으로, 조직의 인슐린 저항성에 의한 인슐린의 상대적 결핍에 의 해 발생하는 제2형 당뇨병(Type 2 Diabetes Mellitus, T2DM)과는 구분된다[1]. 당뇨병의 진단기준은 T1DM과 T2DM이 동일하나 이들을 구별하는 것은 치료계획 수립과 당뇨병 교육을 위한 접근 방법에 차이가 있기 때문이다[2]. T1DM의 주된 치료법은 인슐린 요법으로 인슐린의 절대 결핍 해소라는 근본적인 치료법이라 할 수 있으나 환자들이 어린 나이부터 평생을 인슐린 주사에 의존해야 한다는 신체적, 심리적 부담이 상당히 크다[3]. 이에 대안적 치료법으로 췌도 이식이나 특수면역억제제, 줄기세포를 이용한 치료법들이 제시되고 있다. 그러나 이들 요법의 안정성이 확보되기 까지는 아직 많은 연구가 필요하다[4]. 비록 전체 당뇨병 환자 중 T1DM이 차지하는 비율은 5-10% 정도이나 주로 소아청소년에서 발생하여 이들의 성장저해는 물론이고 기대 수명을 크게 단축시킨다는 점은 심각한 개인적인 문제일 뿐만 아니라 사회적으로도 크나큰 손실이다[5]. 따라서 T1DM의 안정된 혈당 조절을 위해 좀 더 다양한 각도에서 연구가 진행되어야 할 것이다.

당뇨병을 대상으로 실시된 운동 분야의 연구논문들을 살펴보면 대부분 T2DM의 인슐린 저항성 개선을 통한 혈당 유지 및 개선 모색과 기전규명에 관한 것으로 인슐린의 절대적 결핍이 특징인 T1DM을 대상으로 실시된 연구는 상대적으로 적다. 그러나 T1DM을 대상으로 실시된 연구에 따르면 규칙적인 신체활동은 T1DM 환자의 신체 조성 및 심폐기능, 혈중 지질 수준을 개선시켜 심혈관 질환의 발생 위험을 감소시키며[6,7], 일일 총 인슐린 필요량을 감소시키는 것으로 보고되었다[8]. T1DM 환자들의 주된 사망원인이 심혈관 질환이라는 점을 감안한다면 규칙적인 신체활동을 통한 심혈관대사의 개선 또한 매우 중요한 이점이라 하겠다[9,10]. 현재 T1DM 환자들의 건강을 최적화하는 데 필요한 신체 활동의 양이나 강도에 대한 특정 지침은 없다[11]. 그러나 미국 보건복지부(http://www.cdc.gov/HealthyYouth/physicalactivity/ guidelines.htm)에 따르면 당뇨병이 없는 사람과 마찬가지로 당뇨병이 있는 청소년도 하루에 적어도 60분 동안 중등도에서 격렬한 강도로 신체 활동을 하도록 권장하고 있다. 이외 선행연구의 결과들을 보면 운동의 유형은 지구성 운동이나 저항성 운동 모두 긍정적인 혈당 개선효과를 보이며[12,13], 근육 글리코겐과 혈당의 사용을 증가시킬 수 있는 VO2max 50-60% 이상의 운동을 권장하고 있다[14,15]. 그러나 T1DM 환자들의 대부분은 운동 중 발생 할 수 있는 저혈당증에 대한 두려움과 혈당 조절 상실 및 운동시 혈당관리에 대한 지식 부족 등의 이유로 신체활동을 꺼리를 경향이 있다[1619]. 이러한 기피현상으로 T1DM 환자들의 약 60%는 과체중 또는 비만이고, 40%는 고혈압, 60%는 고지혈증을 가지고 있어 심혈관계 합병증의 발생을 더욱 가중시키는 요인이 되고 있다[20]. 이러한 현실적 상황을 고려한다면 규칙적인 신체활동 만으로 인슐린에 대한 의존도를 낮추기에는 어려움이 있어 가벼운 신체활동과 함께 T1DM 환자들의 혈당을 안정적으로 유지하는데 도움을 줄 수 있는 부가적인 방법이 모색될 필요가 있다.

리튬(Lithium, Li)은 현재 조병(mania), 조절이 되지 않고 재발되는 우울증과 양극성장애 치료제로 사용되고 있고[21], 부가적으로 약한 혈당강하효과를 나타내는 것으로 보고되고 있다[2224]. 선행연구에 의하면 리튬은 세포막내 GLUT4 수준을 증가시키나 포도당 유입을 위한 포도당 운반체(glucose transporter 4, GLUT4)의 활성을 증가시키지 는 못하여 인슐린 또는 근수축 보다는 낮은 혈당 제거효과를 나타내는 것으로 나타났다[25]. 지금까지 GLUT4 전위와 GLUT4 활성은 동일한 것으로 생각되어 근초막내 GLUT4의 양적 증가에 비례하여 골격근 내로의 포도당 이동이 증가할 것으로 여겨졌다. 그러나 다수의 선행연구에 의하면 GLUT4의 전위와 활성은 명확히 구분되어 지며, 인슐린과 근수축은 전위와 활성 모두에 영향을 미치는 것으로 보고되고 있다[2629]. 이에 Jung et al. [30]은 고지방식 유도 제2형 당뇨 쥐를 대상으로 리튬과 운동 복합처치가 고혈당을 유의하게 개선시킴을 보고하였다. 또한 동일 논문에서 streptozotocin (STZ) 유도 제1형 당뇨 생쥐를 대상으로 리튬과 인슐린 복합처치가 고혈당을 유의하게 개선시킨 것으로 나타났다. 이들에 의하면 제1형 당뇨 생쥐의 혈당 개선은 세포질내 GLUT4를 세포막으로 이동시키는 역할을 하는 엑소사이스트 (exocyst)의 활성이 증가했기 때문인 것으로 보고하였다. 즉 리튬에 의한 골격근내 exocyst의 증가로 세포막내 GLUT4 분포가 증가하였고, 이때 주입된 인슐린은 세포막내 GLUT4를 활성화함으로 혈당 조절이 유의하게 개선되었다는 것이다. T1DM은 인슐린이 분비되지 않아 혈당이 조절되지 않을 뿐, 조직내 GLUT4의 기능에는 이상이 없다. 따라서 T1DM의 혈당 조절을 위해 인슐린에 전적으로 의존할 것이 아니라 리튬을 통한 GLUT4 trafficking 증가와 GLUT4 activator인 인슐린 또는 근수축을 병행한다면 고혈당 개선에 매우 효과적일 것으로 생각된다. 본 연구자는 선행연구를 통해 T1DM 생쥐를 대상으로 리튬과 저량의 인슐린 병행이 고혈당을 유의하게 개선시킴을 밝혀냈고, 본 연구에서는 또 다른 GLUT4 activator인 운동(근수축)의 병행이 T1DM 생쥐의 고혈당 개선에 미치는 영향을 규명하고자 하였다. 이를 위해 처치 기간은 중강도 지구성 운동에 의한 생리학적/생화학적 표현형의 변화가 유의하게 발생하는 4주[3134]로 설정하여 streptozotocine 유도 T1DM 생쥐를 대상으로 리튬과 중강도의 지구성 운동이 공복시 혈당에 미치는 영향을 관찰하였다.

제1형 당뇨병은 소아청소년에서 주로 발생한다. 때문에 이들과 성장과 평균수명에 부정적이 영향을 미치게 된다. 따라서 이들의 안정적인 혈당 유지는 무엇보다도 선행되어야 하는 치료법으로 보편적으로 사용되는 인슐린 투여에 따른 경제적, 심리적 부담을 경감시킬 수 있는 대체법으로 간단히 복용할 수 있는 리슘염과 가벼운 신체활동이 혈당을 안정적으로 유지하는데 도움을 줄 수 있다면 제 1형 당뇨병 환자들의 혈당조절 및 합병증 예방은 물론이고 삶의 질 향상에 크게 도움을 줄 수 있을 것으로 생각된다.

연구 방법

1. Animals&DM Induction Protocol

본 연구는 10주령의 male C57BL/6J mouse 50마리를 구입하여 1주 간의 환경 적응 기간을 가진 후 정상혈당 비교군 10마리를 제외한 40마리를 streptozotocin (STZ)를 이용하여 T1DM 생쥐로 만들었다. STZ 유도 T1DM은 Wu et al. [35]의 자료를 일부 수정하여 실시하였다. 12시간의 금식 후 50 mg/kg STZ (sigma, #S-0130)를 0.5 M Na-citrate solution (Fisher scientific, #BP-327-1) 400 μL에 녹여 복강내 투여하였다. 투여직후 저혈당에 의한 급사를 예방하기 위해 10% 포도당 용액을 사료와 함께 공급하였다. 5일 후 임의 혈당이 340 mg/dL 이상이 되었을 때 T1DM으로 판단하였고, 기준에 미치지 못한 경우 추가 접종하여 T1DM을 유도하였다[36]. T1DM 발현 후 4집단으로 무선 배정하여 4주간 처치하였다. 사료(Harlan Telkad, USA)와 물의 섭취는 자유롭게 하였다. 생쥐는 한 케이지(20.7×35×17 cm)에 3마리씩 넣어 사육하였고, 사육실의 온도는 21° C로 유지하였다. 명기와 암기를 각각 12시간으로 조절하며 암기는 오전 7:00-오후 7:00로 조절하였다. 실험기간 동안 체중과 사료 섭취량은 3일 간격으로 동일 시간대에 측정하였다. 본 연구는 계명대학교 동물연구위원회의 승인을 받아 실시되었다 (KM2020-013).

  4. Moderate exercise training group (Ex): STZ-induced T1DM, treadmill, 17 m/min×30 min, slope 0%, 5 day/week, n=10

  5. Lithium+Moderate exercise training group (LEx): STZ-induced T1DM, n=10

2. Lithium Administration and Exercise Training

리튬 처치는 Jung et al. [37]과 Khan et al. [38]의 연구 결과에 따라 10 mg/kg LiCl (Lithium chloride, L4408, Sigma-Aldrich)를 300 μL saline에 녹여 운동전 동일시간대(오전 10:00-)에 1일 1회, 주 5일, 총 4주간 경구 투여하였다. Control 집단은 동일양의 saline을 경구 투여하였다. 지구성 운동은 전동식 실험동물용 트레드밀(FT-200, Techman Soft)을 이용하여 주 5일, 총 4주간 실시하였다. 운동 강도는 설정은 Wang et al. [31]과 Yang et al. [39]의 C57BL/6J 생쥐를 대상으로 운동 강도에 따른 생화학적 인자들의 변화를 연구한 결과를 바탕으로 중강도(19 m/min, slop 0%)의 운동강도를 사용하였다. 운동 강도는 초기 10 m/min, 10 minutes으로 시작하여 2주간 시간과 속도를 점증적으로 증가시킴으로 3주차부터는 목표 운동 강도(19 m/min, slop 0%, 60 minutes)로 운동할 수 있도록 유도하였다.

3. Sample Preparation

4주간의 처치 후 last-bout exercise effects를 제거하기 위해 24시간동안 휴식기를 주었다. 이후 12시간의 금식을 실시하고 pentobarbital so-dium (5 mg/100 g bwt)으로 마취하였다. 헤파린튜브(Micro-Hematocrit capillary tube, #22-362-566, Fisher)를 이용해 안와정맥에서 채혈하여 자동 혈당분석기(YSI 2300, Springfield, USA)를 이용하여 혈당을 측정하였다. 채혈 후 조직(soleus, tricep, tibialis anterior muscles)을 적출하여 압착 동결한 다음 분석 전까지 –80° C에서 보관하였다.

4. Western Blotting

족척근(Plantaris muscle)은 ice-cold RIPPA buffer (w/protease inhibi-tor, 4° C)로 균질화 하였다. 균질화된 시료는 동결/해동 과정을 3회(1회는 12시간 동결) 반복하고 원심분리하였다(700 g, 10분). Lowry et al. [40]의 방법으로 단백질을 정량하였다. 시료는 Laemmli buffer로 용해시켜 50 μg의 단백질을 SDS-polyacrylamide gel에 분주한 다음 전기영동을 실시하였다. Immunoblotting시 사용된 항체는 다음과 같다: anti-GLUT4 (#2213, Cell Signaling), PGC-1α (#2178, Cell Signaling), Anti-Rt/ Ms Total OxPhos Complex Kit (#458099, Invitrogen), Exoc7 (#ab118792, abcam), Rab10 (#8127, Cell Signaling), Dynamin (#2342, Cell Signaling), β-Actin (#58169, Cell Signaling), phospho (Thr180/Tyr182)-p38 MAPK (#4511, Cell signaling), p38 MAPK (#9212, Cell signaling), phospho (Thr202/Tyr204)-Erk 1/2 (#9101, Cell signaling), Erk1/2 (#9102, Cell signaling), β-actin (1:1,000, abcam, ab106814). 각각의 2차 항체 사용 후 ECL을 이용해 시각화하고 densitometry (sigma-plot 8.0 system)로 정량하였다. Loading control은 β-actin으로 하였고, 목표 단백질 발현 수준은 β-actin 발현수준에 대해 표준화되어 arbitrary units (AU)로 표시하였다. 인산화에 의해 활성화되는 단백질은 총 단백질 발현량에 대한 인산화 수준의 상대적 비율로 제시하였다.

6. Statistical analysis

각 측정항목에 대한 결과는 평균과 표준오차(Mean±SE)로 산출하였고, 통계적 분석은 SigmaPlot 12.0 통계패키지를 이용하였다. 집단 간 차이를 검증하기 위해 일원분산분석(one-way ANOVA)를 실시하였고, 사후검정은 Tukey법을 이용하였다. 통계적 유의수준은 α=.05로 하였다.

연구 결과

1. Ex training improved blood glucose level in STZ-induced T1DM mice

4주간 NC 집단의 체중은 점진적으로 증가하였으나, T1DM (HC, Li, Ex, LEx) 집단의 체중은 NC에 비해 증가폭이 낮았다(Fig. 1A, ∗ p <.05). T1DM 집단에서는 처치 간 유의한 체중 차이가 나타나지 않았다(Fig. 1B). 12시간 금식 후 공복시 혈당은 4주간의 처치전과 후 모두 NC 집단에 비해 T1DM 집단의 혈당이 유의하게 높았다(Fig. 1C, ∗∗ p <0.01). 그러나 4주간의 처치후 Ex, LEx 집단의 혈당이 HC 집단에 비해 약 40% 유의하게 감소하였다(Fig. 1D, ∗ p <.05). 본 연구의 결과와 유사하게 Al-Jarrah et al. (2010)은 T1DM 쥐를 대상으로 주 5일, 4주간의 중강도(18 m/min) 트레드밀 운동이 혈당과 자궁내막암 표지인자에 미치는 영향을 관찰한 결과 비교군의 혈당(430±13 mg/dL)에 비해 운동 집단(237±10 mg/dL)의 혈당이 약 44% 감소한 것으로 보고하였다. 요약하면 4주간의 중강도 지구성 운동은 STZ 유도 T1DM 생쥐의 몸무게에는 영향을 미치지 않았으나, 공복시 혈당을 유의하게 개선시켰다. 그러나 본 연구에서 리튬 단독 처치는 혈당에 영향을 미치지 않았고, 지구성 운동과의 복합처치에서도 부가적인 영향이 나타나지 않았다.

2. Ex training regulate GLUT4 level and Dynamin in plantaris muscle of STZ-induced T1DM mice

본 연구결과 4주간의 지구성 운동은 T1DM 생쥐의 공복시 혈당수준을 유의하게 개선시켰다. 이러한 현상의 발생 원인을 규명하기 위해 지구성 운동에 주로 사용되는 족척근을 이용하여 생화학적 분석을 실시하였다. 분석결과 골격근내 포도당 유입에 직접적인 역할을 하는 GLUT4의 수준이 Ex, LEx 집단에서 HC, Li 집단에 비해 유의하게 증 가하였다(Fig. 2A, ∗ p <0.05). 또한 GLUT4 세포내 이동(trafficking)에 관여하는 Dynamin의 수준이 Ex, LEx 집단에서 HC, Li 집단에 비해 유의하게 감소되었다(Fig. 2B, ∗ p <.05). GLUT4 trafficking은 세포막으로의 이동과 부착(exocytosis), 세포막에서부터 세포내 저장소로의 회귀(endocytosis)를 의미하는 것으로, exocytosis는 exocyst complex compo-nent 7 (Exoc7)와 RAS oncogene family (Rab10) 등에 의해 유도되고[41], endocytosis는 Dynamin에 의한 것으로 보고되고 있다[42,43]. 아직 골격근의 포도당 섭취시 각각의 상위 신호가 어떠한 방식으로 GLUT4 세포내 이동에 관여하는지에 대해서는 명확하지 않으나[4446], 안정시 세포막내 GLUT4의 수준은 exocytosis와 endocytosis의 변화와 밀접한 관련이 있다[47]. 본 연구의 결과 집단 간 exocytosis 관련 인자인 Exoc7과 Rab10 수준의 변화는 나타나지 않았다(Fig. 2C, D). 요약하면 4주간의 지구성 운동은 골격근내 포도당 이동에 관여하는 GLUT4와 Dynnamin의 발현을 조절하였고, 이러한 현상은 T1DM의 공복시 혈당 조절에 긍정적인 영향을 미쳤을 것으로 생각된다. 그러나 리튬 단독 또는 운동과의 복합처치에 따른 효과는 나타나지 않았다.

3. Ex training increased mitochondria biogenesis in plantaris muscle of T1DM mice

Karakelides et al. [48]에 의하면 조절되지 않는 T1DM 환자들의 골격근에서 ATP 생성률이 떨어지고 미토콘드리아 유전자 발현이 저해되 는 것으로 보고되었다. 이는 당뇨환자들의 혈당조절을 더욱 어렵게 하며, 궁극적으로 합병증을 증가시키는 원인으로 작용하게 되는 것이다[49]. 때문에 당뇨환자들의 미토콘드리아 발현과 기능을 증가시키는 것은 매우 중요하다. 지구성 운동은 인간의 골격근내 PGC-1α 유전자를 활성화시키고, 활성화된 PGC-1α는 미토콘드리아 생합성을 자극한다[50,51]. 이에 본 연구에서는 4주간의 지구성 운동 또는 리튬처치가 족척근내 미토콘드리아 생합성에 미치는 영향을 분석해 보았다. 연구결과 Ex, LEx 집단이 HC, Li 집단에 비해 PGC-1α와 미토콘드리아 생합성 표지 인자(complex II, III, V)들의 발현수준이 유의하게 증가한 것으로 나타났다(Fig. 3A-D, ∗ p <.05). 그러나 리튬 단독 또는 복합처치에 따른 인자들의 변화는 없었다.

4. Ex training and Li administration significantly decrease MAPK family in plantaris muscle of T1DM mice

Mitogen-activatedprotein kinase (MAPK)는 Extracellular signal-reg-ulated kinase (ERK), c-Jun N-terminal kinases (JNK), p38의 단백질을 포함하며, lipopolysaccharide (LPS) 등 다양한 자극에 의해 활성화되면 핵내 전사인자로서 염증 반응을 매개하는 보고되고 있다. Landau et al. [52]에 의하면 T1DM에서 MAPK 신호전달체계가 증가하며 이는 당뇨합병증의 발생과 진행에 필수적인 역할을 하고, 역으로 p38 MAPK 의 억제는 손상된 인슐린 유도 포도당 수송을 부분적으로 회복시키는 것으로 보고하였다[53]. 본 연구의 결과에서도 T1DM 생쥐의 골격근내 MAPK family (p38, ERK1/2)의 활성이 HC 집단에 비해 Ex, LEx 집단에서 유의하게 낮게 나타났다(Fig. 4A-C, ∗ p <.05, ∗∗∗ p <.001). 그러나 Li 처치는 p38을 제외한 ERK 1/2의 활성에 영향을 미치지 않았다.

논 의

본 연구는 지구성 운동과 리튬 처치가 인슐린이 결핍된 T1DM 생쥐의 혈당에 미치는 영향을 규명하기 위해 실시되었다. 연구를 위해 T1DM은 STZ를 이용해 베타세포를 파괴하는 방식을 사용하였고, 지구성 운동과 리튬 처치는 당뇨환자들에게 처방되고 있는 중정도의 운동량/강도와 정신과에서 처방되고 있는 리튬량[5457]을 선택함으로 실현 가능성과 안전성을 높였다. 연구결과 4주간의 처치 후 지구성 운 동을 실시한 집단의 혈당이 비운동집단에 비해 유의하게 감소하였다. 우리는 이러한 현상이 발생한 원인을 찾기 위해 골격근내 생화학적 분석을 실시하였고, 분석결과 운동을 실시한 집단의 골격근내 GLUT4의 수준이 유의하게 증가한 것으로 나타났다. 또한 GLUT4의 세포내 회귀에 관여하는 Dynamin의 발현이 유의하게 감소하였는데, 종합해보면 지구성 운동은 T1DM 생쥐의 골격근내 GLUT4의 양적 증가와 함께 세포막에 머무르는 시간을 증가시킴으로 안정시 혈당 감소에 긍정적인 영향을 미쳤을 것으로 생각된다. 지금까지 운동/신체활동은 인슐린 저항성을 개선함으로 혈당을 조절하는 것으로 알려져 있어 T2DM 환자들에게는 필수적인 요법[5860]으로 여겨지고 있으나, 인슐린이 분비되지 않는 T1DM 환자들에게는 부가적인 요법인 것이 사실이다. 그러나 본 연구의 결과로 지구성 운동은 T1DM 환자들의 혈당조절에 효과적임으로 인슐린에 대한 의존성을 경감시킬 수 있는 대안으로 충분한 가능성이 있다고 생각한다. 본 연구에서 아쉬운 점은 GLUT4 발현을 증가시킨 상위 신호에 대한 규명이 이루어지지 않았다는 점인데, 본 연구는 장기간의 처치에 따른 안정시 혈당의 변화를 관찰하기 위해 최종 처치 후 24시간 안정을 취하고, 12시간의 금식을 거침으로 총 36시간의 휴식이 이루어 졌다. 따라서 자극 직후 나타나는 분자들(예, AMPK, p38MAPK)의 분석은 이루어지지 못했다. 그러나 다수의 선행연구들에 따르면 지구성 운동에 따른 골격근내 AMP/ATP ratio의 증가는 AMPK의 활성을 증가시키고, 이는 GLUT4의 발현을 증가시키는 강력한 인자인 것으로 보고되고 있다[46,61,62]. 본 연구에서도 지구성 에 따른 AMPK의 활성 등에 의해 GLUT4의 발현이 증가되었을 것으로 생각되나 추가적인 연구를 통해 명확히 검증될 필요가 있다.

Peroxisome proliferator-activated receptor γ coactivator-1α (PGC-1α)는 대표적인 보조활성 단백질로 NRF-1, NRF-2와 미토콘드리아 생합성, 지방대사에 관여하는 전사 인자를 활성화시키는 역할을 함으로 미토콘드리아 생합성의 핵심적인 조절 인자로 알려져 있다[50,6365]. 선행연구에 의하면 당뇨환자들의 골격근과 신경세포, 간 등에서 미토콘드리아 수와 기능의 저하가 나타나고 이는 궁극적으로 합병증 발생의 큰 원인으로 작용하는 것으로 보고되었다[49]. 규칙적인 지구성 운동은 골격근내 미토콘드리아 생합성과 지구성 운동능력을 증가시키는데[6668], 본 연구에서도 4주간의 규칙적인 지구성 운동은 T1DM 생쥐의 골격근내 미토콘드리아 생합성을 평가하는 표지인자인 PGC-1α과 미토콘드리아 효소들을 유의하게 증가시킨 것으로 나타났다. 비록 이러한 현상이 혈당조절에 직접적인 영향을 미치지는 인자는 아니나 지구력의 증가를 통한 활동성의 증가가 간접적으로 혈당 조절에 긍정적인 영향을 미칠 것으로 생각된다.

본 연구에서는 리튬과 지구성 운동 복합처치는 단독처치에 비해 혈당 조절에 부가적인 영향을 미칠 것으로 생각하였다. 그러나 본 연구의 결과 리튬 처치에 따른 혈당과 근조직내 생화학적 변화가 나타나지 않았다. 이는 선행연구[25,30,37]의 결과와 일치하지 않는 것으로 발생한 원인은 다양한 측면에서 고려해 볼 수 있는데, 첫번째, 선행연구의 결과를 살펴보면 T2DM 쥐의 경우 리튬과 지구성 운동 복합처치시 단독 처치에 비해 혈당개선의 상승효과가 나타났고, T1DM 생쥐는 리튬과 저용량의 인슐린 복합처치시 단독처치와 비교해 혈당개선효과가 높게 나타난 것으로 보고하였다. 이때 리튬 처치는 단독 또는 복합 처치 모두에서 Exoc7의 발현이 유의하게 증가하였다. 그러나 본 연구에서는 운동의 효과 이외에 리튬 처치에 따른 GLUT4 trafficking 관련 인자들의 변화는 나타나지 않다. 때문에 리튬과 운동 복합처치에 따른 부가적인 혈당강하 효과가 나타나지 않은 것으로 생각된다. 두번째, T1DM 의 혈당 개선은 외인성 인슐린 감수성의 개선과 내인성 항산화제 및 항염증 매개체 증가, 심혈관계의 제어력 증가 등에 영향을 받는 것으로 제시되고 있다[6971]. 선행연구에 의하면 인슐린 저항성의 발달에 관여하는 것으로 알려진 다양한 대사물(포화지방산, 산화 라디칼 및 염증성 사이토카인)은 골격근에서 만성적으로 p38 MAPK의 활성을 유도하며, 이는 인슐린 저항성을 매개할 가능성이 있는 것으로 보고되었다[72]. 본 연구의 결과에서 지구성 운동이 골격근내 MAPK family의 활성을 유의하게 감소시켰는데, 이는 지구성 운동이 T1DM의 혈당 개선에 영향을 미쳤을 것으로 생각된다. 그러나 하위 기전에 대한 분석이 이루어 지지 않아 인과관계를 명확히 할 수는 없다. 세번째, 본 연구는 선행연구의 결과를 바탕으로 실시되었기 때문에 투여량과 운동빈도/강도가 모두 유사하였다. 그러나 실시기간은 12주와 4주라는 차이 점이 있어, 리튬의 효과를 유도하기 위해서는 운동보다는 좀더 기간이 더 소요되는 것으로 추측해 볼 수 있다. 따라서 기간에 따른 추가적인 연구로 이러한 점이 반드시 규명되어야 할 것이다.
